Fees

The fee for Individual Therapy is $175 for a 45 to 50 minute session. Full payment in the form of cash, check or credit card is required at the conclusion of each session. A credit card is required to remain on file for any outstanding balances or cancellation fees. We will charge your credit card on file for any portion of the sessions that you are responsible for unless you specify the desire to pay via an alternative payment method. If a balance is 30 days past due you give Nature and Nurture PLLC permission to charge the card on file the full outstanding balance.

Letter writing, consultations with other professionals, telephone conversations, reading records or reports, travel time, longer sessions etc. will be billed at a prorated rate based on the session fee of $175.

If your account is overdue (unpaid), the card on file is declined and there is no written agreement on a payment plan, Nature and Nurture PLLC can use legal or other means (court, collection agencies etc.) to obtain payment. You agree that in the event Nature and Nurture PLLC is forced to retain an attorney to collect any outstanding balance, you are responsible for the balance plus the attorney fees and court costs in the collections process.

Insurance

Nature and Nurture PLLC is “in network” with Blue Cross Blue Shield PPO, Blue Cross Blue Shield PPO Choice, United Healthcare, and Cigna. As a courtesy to you, we will bill insurance on your behalf. Any balance that is not covered by insurance will be your responsibility, including, and not limited to: deductibles, co-pays, co-insurance, and any denied claims.

It is expected that you will provide our office with up-to-date insurance information and will call your plan directly to verify coverage and benefits. As a reminder, most insurance plans reset at the beginning of each calendar year and deductibles are often due in the beginning months of each year until the deductible is met.

If you are using insurance other than Blue Cross Blue Shield of Illinois, United Healthcare or Cigna, it is your responsibility to pay your therapist the full contracted rate and submit your own insurance claims. Your therapist will provide you with all documentation needed to submit the claims, but you are responsible for full payment if the insurance company denies the claims.

Please note, when filling out our intake information, insurance companies require us to ask your sex and only give us the option of male or female. This does not represent our understanding of sex and gender. We want each client to feel affirmed, so please put your gender identity in the demographic information if it differs from the sex listed on your insurance information.

Cancellation of Appointments

Nature and Nurture PLLC has a 24-hour cancellation policy. For your first late cancellation, the fee will be waived. Afterward, if you miss a session without giving 24-hour advance notice, you will be charged the full session fee. You will be charged the full session fee for all late cancellations and missed appointments. All sessions begin and end on time and your therapist reserves the right to cancel an appointment as a “late cancellation” if you arrive more than 15 minutes after your appointment start time. If you are sick or there is an emergency, every effort will be made to reschedule your appointment to another time during the week. If scheduling works out, we will waive the late cancellation fee. Please be aware that insurance companies do not cover cancellation charges and we will charge your credit card on file.
